Breast lift, technically known as mastopexy, is a surgical procedure to lift and improve the shape of sagging breasts. Normally, the height of the areola–nipple complex is below the breast fold.

Mastopexy can be performed at the same time as a breast enlargement.

There are different techniques: the simpler ones can be done with local anesthesia, while the more complex ones require general anesthesia. All techniques have in common reducing the size of the areola, while positioning the areola–nipple complex back where it belongs.

Types of mastopexy:

  • Mastopexy without enlargement. The sagging breast tissue would be used to create a flap and lift it; that is, the lower pedicle would remain attached to a lower base and would be lifted as high as possible to the pectoralis major. The rest of the breast would be wrapped around it like a corset. For this, a sufficient amount of breast tissue is needed.
  • Mastopexy with enlargement, to increase the upper pole. The enlargement would be done using an implant or via lipotransfer. The sagging tissue would be removed to prevent it from dropping again.

Types of scars for a mastopexy:

  • Periareolar scar, around the areola
  • Donut/pie-shaped scar, around the areola and vertical
  • Inverted T scar: donut/pie-shaped scar plus a small one in the inframammary fold.
  • Anchor scar: as above, but with a longer scar in the fold.

The scar used depends on many factors; in all cases, we try to use the smallest one that still achieves a good result.

The ideal candidates for undergoing mastopexy in Almería are women who have seen their breasts drop and lose volume, whether due to the passage of time, genetics, pregnancy, or weight loss.

As a requirement for surgery, the woman must be healthy and emotionally stable, able to understand the results expected after the procedure.

During the first consultation, Dr. Jaime de San Pío will assess the size and shape of the breasts, the firmness of the skin, and her overall state of health.

An examination of the breasts will also be performed, and in some cases a recent mammography study may be requested (less than 12 months for women over 30). You will be explained the different surgical techniques, discussed what size and shape your breasts will have, and the options—or combination of procedures—that are best for you.

You should share your expectations honestly and openly, so that Dr. de San Pío can show you the alternatives available for your situation, including the risks and limitations of each one.

A small mastopexy can be done under local anesthesia. A large mastopexy, with or without enlargement (implants), is performed on an outpatient basis at the Hospital Mediterráneo. The average stay in the recovery room is 3 hours. If necessary, you may be admitted to the hospital.

You will not need dressings (“curas”), as the incision is protected with paper tape until the stitches are removed. After breast surgery, it is normal to feel somewhat tired for a few days, but you will be able to live almost normally from the first day, thanks to the local anesthesia we use during the procedure.

It is very important to start moving already in the recovery room; this will help us avoid shoulder, back, and neck stiffness, which are responsible for postoperative pain. (video)

Most discomfort can be well controlled with the prescribed medication, although the breasts may hurt for a couple of weeks.

Mastopexy is a safe technique, as long as it is performed by a qualified plastic surgeon. To ensure you are in good hands in Spain, it is important that the plastic surgeon is accepted as a member of SECPRE and AECEP.

However, as with any operation, complications and associated risks can always occur:

  • Bleeding/hematoma: uncommon and highly related to exertion. Only in specific cases does it require a new intervention to remove accumulated blood (less than 0.5%).
  • Changes in sensitivity: Some women notice their nipples are more or less sensitive, and even have no sensitivity. These changes are usually temporary (less than 12 months), although in a small number of cases they may be permanent. It is directly proportional to the initial degree of breast sagging.
  • Infection: very rare thanks to the surgical technique and, above all, by avoiding the use of drains (<0.1%)
  • The remaining complications are specific to implants (see breast enlargement with silicone implants).

Frequently asked questions

After surgery, it is normal to feel somewhat tired for a few days, but you will be able to live almost normally within 24–48 hours. Most discomfort is well controlled with the prescribed medication, although the breasts may hurt for a couple of weeks.

The dressings will be removed the next day, leaving only the skin-colored paper tape (Micropore 3M), which will be removed at the time of suture removal, at 3 weeks. After mastopexy, it can be normal to experience a decrease in nipple sensitivity; this situation is generally temporary, except in some cases where it may be permanent.

Will I have many scars? Normally, the stitches are placed around the areola, in a vertical line downward from the areola, and in very rare cases, along the groove beneath the breast. In some cases, when the breast is small and not very sagging, the incision is placed only around the areola.

After surgery, you will keep the skin-colored paper tape (Micropore 3M) for 6 months, changing it every 3 weeks, and leaving two days of rest after each application. The goal is for the scar to be as thin as possible.

It is very important to avoid sun exposure, especially during the first year, to prevent the scar from becoming dark—meaning to avoid post-inflammatory hyperpigmentation.

The effects of mastopexy, although long-lasting, may not be permanent, depending on factors such as the breast’s initial condition (stretch marks, lack of tissue elasticity), new pregnancies, weight variations, and age.

After a mastopexy, you can return to work in a few days, depending on the activity you do. In the first two weeks, you will be able to move around and lift up to about two kilos. Your breasts will be more sensitive than normal for 2 to 3 weeks, so it may be advisable to avoid excessive physical contact until the 3rd or 4th week.

You can go running starting at around 4 weeks, but we advise delaying sports that involve working the upper body for a period of one and a half months. Start these gradually and moderately.

At Clínica de San Pío, each patient receives a specific recovery program that includes different stretching exercises, as well as other tailored techniques.

Breast massage is not recommended, but shoulder and back massage is

The result will take between 6 months and 1 year to become definitive. If implants are used, the breasts will change during that time, as the periprosthetic capsule forms and the prostheses settle into place to achieve a natural result; the surgery is only the beginning of a process. At Clínica de San Pío, you will have periodic follow-up visits during that first year to ensure the best results.

The ability to breastfeed may be affected in approximately half of the cases.

The only thing that’s certain is that only a reference surgery center, with extensive experience in this field—such as Clínica de San Pío—can ensure the safety and quality needed for such an important step as breast surgery in Almería.
