What does breast augmentation involve?
Breast augmentation using fat transfer (lipotransfer) is a surgical procedure to increase breast size and improve the shape of the chest with autologous fat. When the fat is placed in the appropriate layer, it improves breast projection by enhancing both the shape and volume of the breasts.
Who is it indicated for?
People who want to increase and improve the shape of their breasts, as long as they understand the limitations:
- Cannot be performed on smokers
- Only one cup size can be increased per procedure
- The fat is autologous, meaning you must have enough fat to be able to perform liposuction during the same operating session and then transfer it to the breast (min. 1000 cc of fat).
- The areola-nipple complex must be in its correct position, and ideally the breast should not be very sagging (ptosis).
It is indicated for:
- Enhancing a small or underdeveloped breast with good natural shape
- Correcting asymmetry
- Restoring volume lost after pregnancy or weight loss
- Helping correct congenital conditions such as tubular breasts
If the areola-nipple complex is positioned lower, it may be necessary to perform an augmentation mastopexy (lifting and breast enlargement).
Planning
During the first consultation, the size and shape of the breasts, the firmness of the skin, and your overall health will be assessed. You will be explained the different surgical techniques, and the size and shape your breasts will have will be discussed.
You should share your expectations honestly and openly, so that Dr. Jaime de San Pío can show you the available alternatives for your situation, including the risks and limitations of each.
We recommend a recent mammogram and/or ultrasound (less than 12 months) for patients over 35 years old.
Surgery
It is performed under general anesthesia in the Hospital Mediterráneo as an outpatient procedure, meaning you do not need hospital admission. The approximate stay in the recovery room is 3 hours.
A liposuction or liposculpture of the areas previously determined is performed. The fat is collected in a sterile collector and prepared to be transferred using small cannulas shaped like a spaghetti injector. Micropore 3M tape is placed on the micro-incisions. Only one stitch is placed in the liposuction openings. A bra or any other garment that compresses the breast should not be used during the first month after the fat transfer.
What is the postoperative period like?
You will not need dressings, since the incision is protected with tape until the stitches are removed. After breast surgery, it is normal to feel somewhat tired for a few days, but you will be able to go about almost normal daily life starting from day one, thanks to the local anesthesia we use during the procedure.
Very important: start moving as soon as you are in the recovery room. This will help us avoid shoulder, back, and neck contractures, which are responsible for postoperative pain. (video)
Compression garment in the liposuction areas during the day and night (min. 4 weeks). Stitches are removed at 7 days.
Edema does not completely resolve until 8 weeks. After breast augmentation with fat transfer, you can return to work in just a few days. You will be able to move around and lift up to 2 kilograms in the first week. Most discomfort can be well controlled with the prescribed medication, although your breasts may feel slightly painful for a couple of weeks.
What risks and complications are there?
Fat transfer to the breast is a safe technique, as long as it is performed by a qualified plastic surgeon. However, as with any operation, complications and associated risks can always occur.
- Bleeding / hematoma: uncommon and highly related to exertion.
- Infection: very uncommon.
- Fat necrosis: occurs when more fat is introduced than the area can tolerate. Oil cysts would form and be reabsorbed little by little.
- Sensation changes: very uncommon.
Frequently Asked Questions
Does breast augmentation with fat transfer hurt?
Breast augmentation with fat transfer is very low-pain. The areas that may cause pain or discomfort are those where the liposuction is performed. The most important thing is to follow Dr. Jaime de San Pío’s instructions correctly after the surgery so that the recovery process is as comfortable and painless as possible.
Is the aesthetic result immediate?
The final result will take 1–2 months to be definitive. At Clínica de San Pío, you will have periodic follow-up visits during that first year to ensure the best results.
Will the scars be visible? Where are they placed?
The scars left after liposuction are about 4 mm, and the scars from fat transfer are practically unnoticeable.
